When booking an appointment with Natalie you will be asked to agree to all of the terms and conditions listed below.
YOUR DECISION
Please read this statement carefully:
It is your right to decline treatment and question your technician further if you do not understand anything discussed on this form.
By signing the consultation form document you are agreeing to have read and fully understood this information and therefore indemnify and hold harmless the treating clinician from any liability, cost, damages and expenses arising from treatment.
Once treatment has begun you will be liable to pay the full cost of the treatment - refunds are not available under any circumstances.
As with any treatment it comes with risks.
It is your responsibility to familiarise yourself with these risks in order for you to make an informed decision.
Although good results are expected, it is possible that after the treatment, the face may appear uneven as some areas may be greater affected by the product than others.
It is also possible that the treatment does not last as long as expected.
The results of your dermal filler treatment is dependant on several factors including but not limited to: genetics, metabolism, age, medical history and natural anatomy.
It is because of this that there is no guarantee on the results that can be obtained through treatment.
As with any treatment, dissatisfaction remains a real risk.
Other adverse reactions include but are not limited too: pain, itching, bruising, swelling, inflammation, infection, skin necrosis from vascular occlusion, numbness and scaring. Although unlikely, these may leave permanent damage.
I acknowledge that my practitioner is trained to use a filler dissolving enzyme called Hyaluronidase in the eventuality of vascular occlusion, I hereby give her full permission and judgment to use this product in the eventuality of a complication arising.
In the event of wanting to remove the filler, I acknowledge that fillers can take numerous sessions of dissolving depending on the area of the face, the thickness of the filler and the individual’s natural anatomy.
By signing this document I agree to the above and accept the below information;
I consent to the use of anaesthesia if required.
I understand that photos will be taken and stored for 10 years and that these photos will not be used without my consent.
I understand that this treatment is not always necessary and for the most part is for purely cosmetic reasons.
I understand that dermal filler is charged per vials used and it is the responsibility of the patient to cover additional medical costs should complications occur.
I understand that under no circumstances do we offer full or partial refunds.
In the event a complication arises I understand the clinic is not open 24 hours, it is my responsibility to seek medical help or return to the treating clinic yourself if possible.
Please bare this in mind if travelling from afar.
I had been given the opportunity to take 'cool off' and I now feel in a position to make a sober, rational decision.
I understand that there is no long term studies for new procedures and that if a product is faulty or causes an adverse effect, that the treating practitioner is not responsible for this or any medical bills that may arise as a result.
I understand that If I publish any negative results of my procedures on the internet via social media or other it may result in court proceedings.
I understand that in the unlikely event of an adverse reaction my first point of contact is my practitioner. We will always encourage our clients with concerns to come in and see us for a review first before seeking alternative advice. In the majority of cases we will be able to resolve your problems in house. We advise that you do not book in with another company or clinic (except your GP/Hospital) as this immediately invalidates our review procedure and your insurance policy.
CANCELLATION POLICY
Failure to attend a scheduled appointment and failure to give 24hrs notice of a rearrangement of an appointment will waive your rights to using your deposit towards a different appointment.
If you have not paid a deposit due to the appointment being a follow up appointment/correction appointment, we would charge an extra non-refundable £100 to rebook, reschedule.
LATE ARRIVAL:
If you arrive late to your appointment more than 15 minutes I have the right to refuse your treatment and the equivalent of the deposit payment will be lost.

I acknowledge that paying a deposit/booking fee to Natalie Faccini confirms that I have read and agree to the Terms and Conditions of the company.

Natalie Faccini recognises that personal privacy is an important issue and therefore adheres to the privacy policies in line with UK legislation.
Any personal information gathered through the website or through paperwork signed at the clinic will be used only for insurance and medical purposes. We hereby declare that we do not sell, license or trade your personal information to third parties. We also declare that we do not distribute your personal information to marketing companies or other such organisations.
The pages of this website use cookies for security purposes. These cookies are not used to capture or store personal information for any purpose other than to authenticate the user and are deleted as soon as a session is terminated. Natalie Faccini Aesthetics will generally only receive personal data from the individual concerned directly in the course of conducting business. Usually, personal data collected by Natalie Faccini Aesthetics will remain within Natalie Faccini Aesthetics and will be processed by appropriate employees. Some of the processing is carried by third parties such as website developers, cloud storage providers but is at all times kept securely and only processed with the directions of Natalie Faccini Aesthetics.
On occasion, we will need to share personal information, to meet our legal obligations or for contractual reasons, with third parties such as banks lawyers, insurers, accountants or government authorities such as HRMC if you are an employee.
If you decide to apply for finance we will only share your email address with the finance company so that they may communicate with you directly.
HOW LONG WE KEEP PERSONAL DATA
We are committed to complying with our legal obligation to the retention and deletion of personal information. The type of data and the purpose for collection will determine how long Natalie Faccini Aesthetics will retain your data. We will not process your personal information for purposes longer than necessary.
Employee data can be kept for up to seven years following departure from the company. Details of an unsuccessful applicant to vacancies will be stored for up to one year.
Our insurers require Natalie Faccini Aesthetics to keep Client records, including medical data, images and treatment data for 10 years from the date of your last treatment following which it will be securely disposed of.
Data of those engaged with the business such as contacts for an active contract or receivers of our monthly newsletter will be retained as long as the service is being contracted or newsletter is being sent.
Emails of those who have opted out of receiving our newsletter services will be kept indefinitely to ensure continued compliance.
Financial and accounting records will be kept for 6 years from the end of the last company financial year they relate to or longer if the tax return was late or if HMRC requests it.
